What the UDID card is

The Unique Disability ID, or UDID, is a single national identity card for persons with disabilities in India, issued by the Department of Empowerment of Persons with Disabilities through the swavlambancard.gov.in portal. It combines your disability certificate and a national ID into one card that is recognised in every state.

Before the UDID, a person with a disability had to carry a separate disability certificate and prove it again in each state, often in a different format for central and state schemes. The UDID replaces that with one card and one number, so once you have it you rarely need to re-prove your disability again.

It is the key that unlocks most benefits for persons with disabilities, and it links your record to a central database that many schemes read directly.

Who can apply

Any Indian citizen with a disability recognised under the Rights of Persons with Disabilities Act, 2016 can apply. The Act covers 21 conditions. You do not need an existing disability certificate to begin, the process issues one as part of the assessment.

How to apply, step by step

Register on the portal

Create an account on swavlambancard.gov.in or the UDID app with your name, mobile number and email, and verify the OTP.

Fill the application

Enter your personal, disability and contact details, and select the disability type that applies to you.

Upload your documents

Attach your photograph, identity proof, address proof and signature. Blurred scans are the top cause of delay, so check each one.

Save your enrollment number

On submission an enrollment number is generated. Save it, you need it to track status and download the card later.

Attend the medical assessment

Your application goes to the District Medical Authority or CMO office. Attend the assessment at the assigned government hospital, where the board assigns your disability percentage.

Receive your card

On approval your disability certificate and UDID card are generated. Download the e-card from your PwD login, the printed card arrives by Speed Post.

Validity and renewal

How long the card lasts depends on the disability and the age of the holder. For an adult with a permanent, non-progressive disability the UDID has lifelong validity and never needs renewal.

Children under 18, and anyone issued a temporary card, renew every five years, because a child condition can change as they grow. Apply for renewal a little before the expiry date and you keep the same UDID number.

You can renew or update from the PwD login on the portal. A fresh medical assessment is usually needed only if your condition or its percentage has changed.

UDID card versus the disability certificate

People often ask whether they need the disability certificate or the UDID card. The honest answer is that the UDID process gives you both. The medical board issues your disability certificate, and the UDID card is generated from it.

The certificate proves your disability and percentage. The UDID card is the portable national version of that proof, with a unique number linked to a central database. Most colleges and offices still accept the certificate, but more institutions, especially central scheme ones, now ask for the UDID card, so the card covers you everywhere.
